This application is to be completed by all those desiring a ministry position involving the supervision or custody of minors. It is being used to help the church provide a safe and secure environment for those children who participate in our programs and use our facilities. This application will be treated with confidentiality. Please be aware that you will be required to have a criminal background, child abuse, and sexual abuse checks completed to serve in Children's/Youth Ministry.

Large Group Leader (Present lessons in an engaging and effective way, being comfortable in front of a large group of children and adult leaders.)

Small Group Leader (Reinforce lessons with the children in small groups through activities, connect with children on a deeper level through prayer, discussion questions, games, etc.)

Classroom Assistant (Provide support for the Large Group and Small Group Leaders as needed through gathering supplies, engaging kids in the lesson, connecting children with one another, and modeling worship and active listening.)

APPLICANT'S STATEMENT

The information contained on this application is true and correct to the best of my knowledge. I authorize any places of ministries listed to give GBC information that thy may have regarding my character and fitness to work as a volunteer.

Having committed to the ministry of teaching and the habits essential for spiritual maturity, I commit to:
1. Prepare for ministry by maintaining my personal relationship with Christ;
2. Support the teaching ministry by praying for the church and GRACEkids staff, the other volunteers, and, specifically, the children in my class;
3. co-operate with other ministries and place the greater good of the whole body over the needs of my ministry; and,
4. continue my personal growth and education by participating in ministry training.

I understand my service commitment is from September to August each year, and I will communicate directly with the Children's Ministry Coordinator if circumstances arise that prevent me from meeting that commitment.
